Have you consulted the Game or Client Crashes topic in this subforum? Here's the link.
To quote Speeder, one of the developers:
But before you report them, make sure you did everything as http://mentalomega.com/install said.
I suggest you head there and see if any of the troubleshooting tips help resolve your problem.

Iam aware of that the problem might be, that there is no gamemd.exe - but I didnt know that the XWIS copy does not have that file. So i thought maybe I overlooked something.
Thanks for your quick replay tho.

For the sake of completeness, there are other reasons the "Fatal Error: Could Not Load Executable File" error could pop up.
Even though you, in this case, were missing the gamemd.exe completely, it is also possible that you do have gamemd.exe but access to it is somehow prohibited. A friend of mine had this issue yesterday.
If you get this error but you are sure the gamemd.exe exists and is in the right folder, be sure to try the following:
If this does not fix your problem, try disabling your antivirus / internet security software. If that does solve it, you could consider enabling the software again and fiddling around with your antivirus's whitelist.
In my friend's case, he had to unblock gamemd.exe to get Mental Omega to work. As far as I know this has not been mentioned anywhere else, which I guess makes sense since you wouldn't necessarily get those files directly from the internet as opposed to via a (downloaded) installer. Still, this might help others experiencing the same issue.

What about point 3? Did you run it using the admin + compatibility settings?

Is your game installation legit and uncracked, as well as on the right version (1.001)?

Another issue I've experienced was that the gamemd.exe just doesn't launch; It doesn't give me any error it just sends me back to the launcher..
I tried everything. I changed the compatibility settings, I made it run as administrator and nothing works except restarting my pc, it only sometimes work.
