I think it'd look a little better if the skulls in the fountain itself weren't moving in circles. Maybe if it just appeared as if they were slowly rising up and down in the blood...
Not sure if it should be a healing structure, though. In the interests of faction diversity, I think its best for the evil factions to focus on things like damage, fear, and debuffing enemy forces while the Good ones have the healing and defense powers & buildings.
I like it, but I think it could serve a better purpose than a healing structure.

but like dzhon said there shouldnt be a healing abilty ... on the otherhand you could make it like the statue but give a fear ability

It's not a huge deal either way, but the idea of Orcs tending to their wounded sounds a little strange. The only units Mordor needed to keep alive were Mumaks, Trolls, and their Rhun and Harad allies (all of which now auto heal at level 2 or woundn't drink blood.
Orcs are more or less designed to be cheap (even free) and dispensible. The build in large hordes, so regeneration can keep them going. I suppose they could even heal if kept out of combat for a time like the Elves do (my general impression is that wounded Orcs would either be patched up quickly using primitive techniques or just left for dead.
As I said, though, great structure...I'd just have it debuff enemy units or something like that. Fountains and Heroic Statues are a Forces of Light thing imo...just like the "Bloodthirsty" ability and Fear structures are Forces of Darkness.

ps: you true, orc have BIG hordes (Orc fighter - 50 orcs ,damage - 3 health 25,cost - FREE. ORc Archers - 50 orcs ,Damage - 4,health 20,cost - 150)
i have 1 idea -maybe creat "Slave Master" unit (Like WOTR game) example: you have 1 slave master - you can build 2 hordes of orc fighter and 1 hordes of orc archer?(maximum population for "Slave Master" - 5)

2)maybe add some sorts of "Leaders" for any factions? Example Gondor:Guard of Dol amroth (very powerfull unti with heavy armor and can switch bow/sword) Elves: healing elfs? (maybe elf wizards or etc?) Mordor: Slave master$) Izengard: Uruk leader (like helms deep in movie), and etc

do you remember the Elven Lightbearers from WotR cause they would be a good unit to use if your looking for a mini hero for elves
