Here's the template. I took most of it from other RP templates...

Story: Dark Lord of Derkholm
Weapons: Soul-bound Demon servant
Appearance: Tall, balding
Biography: Mr. Chesney uses Derk's world as a tourist attraction.

Obviously I'll want longer biographies than the ones I posted there, so be somewhat detailed. Still, leave room in your character for some development; static characters aren't much fun.
