In Mirkwood, Galadriel and the Lorien army arrive shortly after you approach Dol Guldur and the Mordor army attacks you. At the moment, you can win the match without destroying Dol Guldur, which might be why you haven't seen Galadriel all the time (if you detroy the camp before Dol Guldur).

Dunharrow, Osgiliath and Rhun were planned to have changes made to them, but Cel didn't get around to it.

No, you misinterpreted my question. I had a save in which I had Galadriel at the Black Gate, for she came along my Gondorian army since Mirkwood. I wondered how I achieved this.

On a side note: the site states that no elven units come along with your army after Mirkwood, but it is fairly easy to have Mirkwood scouts come along. If you have 30CP left, 2 battalions will join your forces after the mission. I've got both of them at level 10 right now :p Pure own.
