We would see millions of refugees, but we've seen that sort of stuff before. Tsunamis have displaced such numbers before, aid-organizations have been able to help out in those cases. I don't think SK's economy would be hit badly if they can avoid a saturation-shelling of Seoul.
Alas, it seems that the risk of escalation has died down a few steps now, at least until the new Joint training op with the US and the USS G.W. on it's way, NK might respond poorly and make another mistake. Most likely they'll use this event as leverage in another diplomatic meeting, which people should see through because this is the only way they can get diplomatic clout to get anything going for them; blowing up shit and pretending they are sorry and for X they won't do it again...for now.

...It's like The Cold War all over again. Except this time instead of Russia, it's China we have to deal with. And it doesn't help that certain Chinese officials see North Korea as their East Germany. This could get messy.

There are probably plenty of Chinese one-feather-chieftains that would do their best at keeping NK around the way it is today, but I dunno how much they have in the say if NK keeps doing idiotic things. The most interesting question is how China would respond if NK got attacked big-time by SK+allies. I'd say they might try to invade from the north and make a "North-North Korea". Then it probably would become a race for Pyongyang and who knows what might happen if both got there at the same time...
