Here is an updated multiplayer guide, since I don't think any other has been updated for a long time:
Steps you should take:
1) Make sure you both have same game or mod version. So for example if you have version 1.2, then make sure other player does. Or 1.0, 1.46 etc.
2) Download and install either Hamachi or Tunngle (Links below). Tunngle probably better for 1.0/1.2, but Hamachi better for DP.
3) Create a Hamachi/Tunngle account depending which you installed.
4) For Hamachi: Join a network with your friend(s), or create a network and invite your friend(s) to join the same network.
For Tunngle: Join the Commandos 2 room.
5) Turning off windows firewall when playing online is highly recommended, especially for Hamachi.
6) For Hamachi: Start the game, choose multiplayer, then choose (TCP/IP INTERNET), put your Hamachi IP then create the game, get your friend(s) to join by putting the same IP.
For Tunngle: Start the the game, choose multiplayer, then choose (TCP/IP LAN), and the host will create game from there.

Tips:
Press ctrl+enter to chat in game.
Use ctrl+s for save and ctrl+l for load.
Try to avoid using Green Beret's decoy as that is the biggest cause for sync problems because of random behaviour.
If you run into a sync problem, you can fix by sending your friend(s) your save file (QSRxxxx.sav from OUTPUT folder), and have them install it to the same folder.
If not working, some other things you can try are:
Disable anti virus software
Go into your router settings and disable UPNP
Forward the ports 2300-2400 (TCP&UDP), 47624 (TCP&UDP) 11155 (TCP&UDP)
